Too late! I submitted mine months ago.
Besides, the IRS already has their hand in my pocket. And the FBI created a file on me when I joined up Uncle Sam's Big Green Machine. As far as the CIA goes... they have an eye on everybody anyway.

They won't send it in the mail - too expensive. You might get an unexpected visit from someone out there returning the courtesy and requesting to leave his/her name with you on our planet.
